In the event of the birth or adoption of a child, you may wish to
designate your child as a plan beneficiary. To do so, complete
and return an ACERA
Active or Deferred member Beneficiary Designation Form. In
addition, while not required, you may wish to provide ACERA with
a copy of your marriage certificate and your child’s birth
verification. This information will be needed in the event of
your death, for benefit payment to your child.
